Workshop Leader Application
In order to be considered for an internship, you must submit this completed application form along with a letter of recommendation from a high school faculty member, school counselor, advisor, or your employer. Letter of recommendation should be mailed to CPPAC, P.O. Box 31094, Portland, OR 97231, or emailed to info@cathedralparkarts.org. The deadline has been extended and all application materials must be received by June 6, 2018.
