For a while now I've been thinking about adding a "disliked members" question to the surveys.
I've been hesitant to add it mostly because of the potential controversy but also I have some concerns.
Though recently I've had reasons to think it's inclusion might be positive.

I wont give any arguments because I don't want to overly influence your responses so please be honest.
Also, I like to think of these surveys as being community-owned, so please don't hesitate to state what you want.

If added:
The question will be optional (they all are, always)
It will specifically ask for actually disliked members, not just members you don't care about.
As well as the option to ignore the question, there will be an ~"I don't dislike any members" response option.
When being published there will be some kind of distinction to help those who don't want to see avoid seeing the results. A "trigger warning" of sorts.
