Patients from the practice have formed a Patient Participation Steering Group (PPG) to assist and support the medical staff with improving the service currently available.

Patient Survey

Members of the PPG, with the backing of the medical practice,  have compiled this survey to help identify the quality of care you currently receive. By taking a few minutes of your time we will be able to identify what is important to you.

All survey responses are anonymous and the results of this survey will be handled and analysed by members of the PPG Ongoing results can be viewed by you online after you have submitted your survey. The final results will displayed will be in the surgery, on the practice website and shared with the medical team to identify areas that need improvement.

If you are completing a hard copy please post the form directly into the collection box.

Please note no questions will be responded to directly and all the information kept securely and used lawfully in accordance with the Data Protection Act.
