Per MSU policy, all volunteers in youth programs are required to complete and sign a background check form. Please make sure to sign on the signature line. You can either scan it or take a good picture of it and email it back to Sandhya (shankers@msu.edu). Alternatively you can stop by our office in B135 Wells Hall and request to fill out a hard copy of the form.
