GMHC Member Application Fall 2014
Thank you for your interest in the Gay Men’s Health Collective (GMHC)! The GMHC, a division of the Berkeley Free Clinic, was founded in 1976 to provide free, non-judgmental STI services to gay men in a time when alternative sexual practices were still heavily stigmatized by the medical community. Today, the GMHC continues to care for our own but have also expanded our services for all male-bodied individuals. Because of our history and our desire to maintain a safe space, we ask that our volunteers identify as gay or bisexual males. Every year, we do a training for volunteers interested in becoming members and medics within our collective. This application will only be for membership. Medic applications will be given to those who complete the member training.

Member Privileges
Members are eligible to work in non-medic volunteer roles, and have full voting rights. Membership training occurs in the Fall 2014. You also need to be a member to apply for medic training.

Medic Privileges
Medics provide direct service to clients of the GMHC. To undergo medic training, you must have completed the membership training. Medic training will occur in Spring 2015. Medics must be at least 18 years old to apply for medic training.

These following questions are designed to help us gain insight into you as a person. We are interested in your personal experiences and reflections; we do not have preconceived in mind!

We also request submission of a personal photo along with your application.  This will help the members of the Training Collective better associate each application with its respective prospect, as your application is reviewed.

APPLICATION DEADLINE: Friday, September 26, 11:59PM.
NOTE: We recommend you write your responses in a word processor and then copy them into the application!
