RD2L S5 PST-FRI Replay Submission Form
Please make sure you download the replay (a .dem file) from your dota client (...\Steam\steamapps\common\dota 2 beta\dota\replays\) and then upload it to a file sharing site (http://www.mediafire.com).

Also make sure you screen shot the score screen (either at the end of the game or from your match history from your dota profile) and upload it to an image sharing site (https://imgur.com/).

You will need to fill out this form twice, once for each game of the best of 2 match.

(This should be the same number that is the filename of the replay followed by a .dem)
