     Swim practice - M-F  5:30pm-7:00pm Chapel Hill Community Center pool (120 South Estes Dr.) 10/29-10/31 is 6-7pm
     4 practices required per week
     Athletes responsible for own transportation to practices

Swimmers practice 5:30-7:00pm at the Community Center pool off of Estes Drive. I am aware that swimmers provide their own transportation to practice. *

High school swim meets are run by parents. I am aware that I will be asked to volunteer at meets, meet preparation, dinners, projects, etc. I am willing to help occasionally. *

If there is another CHS swimmer or diver in my family, I will need submit a form for each athlete. *
I am aware that this is not an official registration through CHS. I need to register through the Athletics webpage ( http://nc.8to18.com/Carrboro/newsfullstory?newsid=82844)  and need to submit a current physical form to CHS Athletics before I can practice. *
