In the interest of having accurate information available to all parties, Westside School will appreciate you providing the following information when you are in the process of planning an event. In order for the facilities team at Westside School to help you successfully pull off your event, this form must be completed.

PLEASE NOTE:
* Calendar coordination is necessary for all events, contact Communication Coordinator Shoshannah Hoffman to ensure information is on the Westside School calendar (shoshannahh@westsideschool.org)
* You must complete this request four weeks in advance of your event
