Participant agrees to the following conditions. Participant understands that failure to follow these standards will result in them being asked to leave the camp.
• I understand that Camp fYrefly is a physically and emotionally safe environment.
• I will not use alcohol, drugs, or engage in sexual activity during the camp.
• I have read and agree to fYrefly’s Smoking Regulations.
• I have read and agree to fYrefly’s Electronics Policy.
• I will treat others and myself with dignity and respect.
• I will respect the space and rules of the camp facility and University of Alberta.
• I will be present and actively participate in all camp workshops and events to the best of my ability.
• I agree to fill out an evaluation of my participation in the camp, which may be used for research purposes to help us improve the camp in future years. The information I give on this evaluation will be anonymous and confidential.
• I understand that media personnel may be present at the camp and that my participation in any interviews is completely voluntary. I understand that media participation is not a requirement of camp attendance.
• I am responsible for my own health and wellness, which includes adequate sleep and nourishment.
• I will not be permitted to leave the Camp for outside social or recreational purposes, which includes visiting local friends or going to bars or clubs. I also understand that friends who are not attending the camp will not be permitted to visit me while I am at camp.
• I authorize the Camp Directors & Staff to secure medical treatment for me when appropriate including occasional administration of over-the-counter medication as necessary, but not limited to medication, x-ray, hospitalization, anesthesia or surgery. If for any reason I receive such medical attention or special medication beyond that furnished by the camp, I agree to be responsible for all expenses incurred.
• I will inform Camp Directors and/or Camp Coordinator about any issues or problems that arise for me at camp.
• I acknowledge that I will be attending the camp at my own risk. I hereby waive any right of recourse I now have or may acquire in the future against the Governors of the University of Alberta, its employees, volunteers or agents or anyone else associated with Camp fYrefly if I sustain bodily injury, personal injury or property damage, except where such injury or damage is due to the negligence of the University of Alberta, camp organizers, the Camp site facility, or other adult facilitators associated with the Camp.

The Camp Directors reserve the right to dismiss a person from camp and to send them home at their own expense at any time and without notice when it is deemed to be in the best interest of either the person or the camp, or for the violation of any camp rule/agreement, all as determined by the Camp Directors in their sole and absolute discretion.

Conditions of Acceptance to Camp fYrefly
While Camp fYrefly makes every reasonable effort to ensure that all participants will be supervised, kept safe, cared for, and nurtured during their Camp experience, participating in Camp fYrefly does pose some potential inherent risks. As such, you must agree to the following terms and conditions for participation:

DISCLAIMER
The Governors of the University of Alberta, their agents, offices, directors, employees, volunteers, contractors, servants or representatives (hereafter referred to as the University) are not responsible whatsoever for any death, injury, loss or damage of any kind suffered by any person while participating in the above noted program.

ASSUMPTION OF RISK
I am aware that there are risks associated with my participation to the above noted program and related activities at the University of Alberta, Edmonton, Alberta, which include but are not limited to:
1. Injuries or death resulting from travel to and from Camp site locations by public, commercial or private motor vehicle or aircraft;
2. All manner of muscular injuries, bruises, scrapes, cuts, sprains, dislocations, broken bones, and head, facial or dental injuries;
3. Injuries from touring the University of Alberta Campus, staying at Camp site facilities, visiting the Princess Theatre, participating in guided outdoor adventures, or other associated field trip;
4. Extreme changes in weather or temperatures which may result in heat-stroke, sunstroke, or hypothermia;
5. Allergic reactions to food and/or medication;
6. Exposures of an adverse nature (such as harassment, sexual harassment, physical confrontations, etc.) that could result in mental and/or physical damages or injuries as a result of activities that I may engage in during the unsupervised free time. This could include exposures while traveling to and from locations of sight seeing, shopping, movies, exposures from where I may be staying at other than U of A residences; and
7. Medical problems arising before, during or after participation in the above noted Program.

ACCEPTANCE OF RESPONSIBILITIES
The participant understands and acknowledges the following:
1. To select and purchase adequate travel and/or medical/health insurance. No travel and/or medical/health insurance will be provided by the University of Alberta. In the event of a medical/health problem, the University of Alberta accepts no responsibility for any costs associated with a medical/health problem nor will they pay for any medical/health expenses, which may be incurred by the participant.

2. The participant will obey all the rules and regulations pertaining to the above noted program and all related activities.

I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD THIS AGREEMENT, that I understand, appreciate and accept the risks associated with my participation in the above noted program and related activities at the University of Alberta. As the participant, I agree to abide by these terms/ consent for my participation in the above noted program.
