This application is for those Arrowmen whom are interested in earning the Lodge Service Flap. There are some minimum requirements to qualify for one of these prestigious flaps.

1. You must be a registered member of the Lodge with current dues paid
2. Perform a minimum of 40 Service Hours during the Calendar year. (You can earn a maximum of two (2) service flaps a year. Qualifying hours include (Unit service projects, Lodge service projects, Ceremonies, Staffing events, Community service projects)
3. Fill out this application with the Service you have provided and you will be on the recipient list for the next Lodge Banquet
