Smith Holiday Parties

Please complete a form for each person who will be attending the Holiday Party. If you have more than one student at Smith, please use the oldest student’s name and teacher when pre-registering. All parents will need to check into the office to pick up their “Holiday badge” before attending the parties on Friday, December 18th at 1:45p.m.
Please be aware that all forms MUST be submitted by 10:00 a.m. Friday, December 18th.

All people who have not had their license scanned into the Raptor System this school year will need to stop in the office and have their license scanned into the Raptor System before entering the building and attending the Holiday Parties.
