2017 ITA DI Men's Arthur Ashe Jr. Leadership & Sportsmanship Award Nomination Form
**NOTE: all ITA Men's Arthur Ashe Regional Winners will be considered for the ITA/Rafael Osuna National Award.  Please submit any at-large candidates for the Osuna National Award to Mary Edman (medman@itatennis.com)

Award Eligibility:
ITA Junior or Senior Student-Athlete who has been enrolled at his/her current school for at least one year.
Candidate Letter must be submitted to the Regional Chair.
The pool for ITA National Award Winners comes from the Regional Winners plus any at-large candidates.  

Nominees will be evaluated on:
Singles & Doubles performance during the current year as well as overall playing career
Sportsmanship, Leadership, Character
Scholastic Achievements (GPA, Awards, etc.)
Extracurricular Achievements and Honors
Candidate Letter

Selection:
ITA Regional Awards Winners are selected by ITA Regional Committees.
ITA National Awards are selected by the ITA National Awards Committee, which is comprised of one representative from each Region along with the Chair and Associate Chair of the DI Operating Committee
