Dear Centre Ridge families,

As we start planning for next year, if you would like to share information regarding class placement, please complete this optional survey to share your input regarding your child's learning environment. While we will take all requests into consideration, it may not be possible to honor every request.

Knowing the importance of your child's continued success at CRES, we will do our best to place your child in the best learning environment possible for their success for the 2016-2017 school year. We will use this input as we work with staff to finalize class placements over the summer. Please use this form to make your requests.

You must fill out a separate form for each child. The deadline for submission of this form is Monday, August 1st. We will be using the information submitted as we work to finalize class placements.
