UPDATE: MAY REQUESTS ARE NOW CLOSED.  No further requests will be pulled from the database.

The following form will allow you to submit your requests for WEEKEND days off as well as if you would like to be post on a certain day for a valid reason.  

Please submit the form for each day you are requesting off.  If you are requesting 2 days off (i.e. a weekend, please submit a request for each day).

All requests are due 6 WEEKS PRIOR to the start of the month (e.g.  July 15 for September 2016).  
Special Cases
For July- June 5

Keep in mind, all REQUESTS ARE ONLY REQUESTS.  If they can be granted, we will attempt to do so; however we have to maintain compliant coverage.  Requests submitted after the 6 week deadline will generally not be considered as the requests will have been pulled from the spreadsheet.

If you are submitting MULTIPLE REQUESTS in one month (not a single weekend, but two different weekend days), indicate which is the priority (in the last question) if both cannot be given.  It is highly likely that you will NOT get both requests.  


PGY IV ELECTIVE
If you are completing your elective month at Sinai (e.g. pathology, additional laparoscopy), you will be expected to take senior call.  1 day per week, weekdays only.  You may submit a requests for the days you would like to work.  Simply indicate this is for your elective month in the last response item.


PGY II BUDDY CALL
If you are on a service that takes buddy call (anesthesia, pathology, endoscopy, thoracic, radiology), you can submit a requests not to be on buddy call on a certain Friday night.  NOTE that the ability to grant these is very dependent on how many PGY2 are available for buddy call each month.  

For example, some months have 3 people-this makes it easy to accommodate. Some months have 1 person-this means you are the buddy each Friday night and no requests can be granted.
