Prestwich Marauders Girls Development Membership, Medical & Parental Consent Form
In ‘signing-on’:

1. We become members of Prestwich Marauders F.C. and are bound by the Club Handbook. (See Link at bottom of the form) We have read the handbook and understand our responsibilities as parent/guardian(s) and as players. We will abide by the codes of conduct, the child protection policy, the equal opportunities policy and any other policy brought in by the club.

2. We are aware of the FA's Respect Guide and agree to support it (http://www.thefa.com/respectguide/).

3. We are aware of the Club's Privacy Policy (http://www.prestwichmarauders.co.uk/wp-content/uploads/2019/03/Privacy-Notice-Prestwich-Marauders-FC.pdf) and Data Protection Policy (http://www.prestwichmarauders.co.uk/wp-content/uploads/2019/03/Data-Protection-Policy-Prestwich-Marauders.pdf)

4. We give consent for player photographs to be used in publications promoting Prestwich Marauders F.C. and on its website. First names only will be used in identifying individuals.

5. We will make sure that our signing-on fee and subsequent subscriptions will be paid on time. As members of the club, Parent/Guardian 1 (and Parent/Guardian 2 if declared) take(s) full responsibility for any money owing.

6.     We acknowledge that any fines levied on the named player by the Club or League are payable by the player/parent and NOT the responsibility of the Club. As members of the club, Parent/Guardian 1 (and Parent/Guardian 2 if declared) take(s) full responsibility for any money owing.

7.     We acknowledge that all kit remains the property of the Club and we agree to return any kit when requested by the Club.  Should kit not be returned when requested we agree to pay the full cost of replacement to the Club.

8.     We give consent to a qualified first aider to administer first aid and/or seek medical help from emergency services, in the event of an injury, sickness or accident to the named player, during or in connection with any activity relating to Prestwich Marauders FC.

9.     We consider the named player healthy enough to participate in team activities.

10.   We have ensured that the named player has adequate tetanus cover at least for the duration of the coming season.

11.   We give consent to the named player travelling in another parents/guardians vehicle in accordance with the club policy on child protection.

12.   We have supplied emergency contact details of people who are authorised to make decisions in the best interests of the player and are immediately available.
 
13.   We understand that it is our responsibility to update Prestwich Marauders FC should any of the emergency contacts change or the players medical information changes.
