2016 CORE/O'Fallon Parks & Rec 3v3 Halloween Spooktacular
Welcome and thank you for registering for the 2016 CORE/O'Fallon Parks & Rec 3v3 Halloween Spooktacular. CORE Soccer Training and O'Fallon Parks & Rec have teamed up to put on their first 3 v 3 Tournament specifically for the O'Fallon League. This tournament is designed to show off the skills developed in O'Fallon League play, as well as have the end of season bragging rights. Each team MUST be comprised of ONLY players who participated in the 2015/2016 O'Fallon League (Spring or Fall). Existing teams are encouraged to split their teams up to form multiple teams for this fun and exciting end of season event.

Tournament Location and Dates
O'Fallon Sports Park, O'Fallon, MO. - October 29th (ONE day Tournament)
Check in MUST be completed 1.5 hours prior to your first game)

AGES: U6-U14 Boys, Girls or COED (COED teams will play in the Boys Division)
Maximum of 6 PLAYERS per Team
ALL teams will be placed in their requested divisions
* ONLY exception - if there are not enough teams in a division, we will place you

COST: $150per team (maximum six players, minimum three players on a roster)
EARLY Registration (SAVE $25 if you register and pay by October 15th - COST $125)

PARKING WILL BE FREE FOR THIS TOURNAMENT

REGISTRATION INSTRUCTIONS:
1. Complete the tournament registration form below and submit it for processing. Once you have submitted this form, an on line registration confirmation page will be generated. This will be your receipt for our 3 v 3 Tournament. Print out this blank form and MANUALLY fill in the additional information that is requested. This form will be part of your Team Tournament check-in Packet.
 2. Once Registered, the Tournament Directors will send via email, your Official 3 v 3 Roster form along with the Tournament Rules. Fill in ALL the information on the roster form, including a parent and turn your roster in during check in.
