Welcome to the Nevada Department of Public Safety / Nevada Highway Patrol online traffic crash report request form.  Please follow the following steps:

1.  Provide as much information as you have about the crash report you are requesting.  The more information you can provide, the better chance of us finding the crash in question and servicing you.

2.  Complete the "Requestor Contact information" on the form

3.  Wait for contact from the Department of Public Safety / Nevada Highway Patrol (typically within 1-3 business days) to    arrange receipt of the report, which may take 7-10 business days.
