In an attempt to reduce paper and get this information to our classroom teachers as quickly as possible, we are asking each family to complete this survey for each child that is attending the Burke School this year.  We ask that families complete this form prior to the first day of the  2015-2016 school year.  We greatly appreciate your cooperation with this as we work to ensure smooth dismissal transitions for all students.
