Thank you for taking part in our survey.  We run this survey once a year, before our strategic planning meeting.  The responses you give to the questions will help the management and partners of Westcare decide how to improve and expand the service we provide.  Your feedback is a very important part of this process, whether you work with us or use our services.

The answers you give will be anonymous, and will remain confidential.  The partners and management will discuss the results of the survey, but will not discuss who made the responses.  The responses you give will not affect the care you receive at Westcare, or your employment with us.
