I acknowledge that I must carry proof of age and eligibility with me at all times and be ready to produce this proof if asked by the referees or tournament officials and/or staff.  Proof of age and eligibility consists of laminated player cards with pictures issued by either USYSA or US Club Soccer.  If you do not have player cards then you will need to provide proof of registration in the form of a letter from your club registrar certifying that your player is indeed registered and eligible for the 2017/2018 seasonal year.  The letter should be on club stationery and include team name, player name, player date of birth, and player ID number.  This letter must include all players for which you do not have player cards.   Failure to provide proof of age and eligibility will result in a forfeit. *

I acknowledge that all game cards must be turned in to the tournament headquarters tent within 30 minutes of the end of the game.  The game card is to be turned in by the winning team or the home team in the case of a tie.  Failure to abide by this request may result in a forfeit of the game. *
