Utah Glory Juniors Winter 2015 Indoor League Registration
U9 and under co-ed Indoor Soccer League hosted by Utah Glory Soccer Club. All games will be played on Fridays/Saturdays at the Utah Glory Indoor Training Facility located at: 220 West Cottage Ave (8755 South) in Sandy. Games will be two, 20 minute halves with 6 players (5 plus a keeper). Cost is $250/team per session. Age groups will be divided as U6/U7 and U8/U9.

I, as the listed coach/manager of the players listed on this form, give permission for our team to participate in the Indoor league hosted by the Utah Glory Soccer Club. I, on the behalf of myself and players listed on this form, release and indemnify Utah Glory Soccer Club, its employees and other personnel against any claims, damages, or liabilities brought on as a result of the player’s participation in the Utah Glory Soccer Club indoor league. I also grant my authorization and consent for Utah Glory Soccer Club personnel to administer general first aid treatment for any minor injuries or illnesses experienced by the minors listed. If the injury or illness is life threatening or in need of emergency treatment, I authorize the personnel of Utah Glory Soccer Club to summon any and all professional emergency personnel to attend, transport, and treat the participant.
