1) You can submit only ONE response to this form. Multiple responses are disallowed.

2) This application does take into consideration the IP address of the applicants and hence the rogue applications would not be entertained.

3) This form contains multiple pages. Please read and fill out the entire form carefully before making a submission.

4) This is an individual delegate application form. There is no separate application for registering as a Delegation.By default, when the number of individual delegate applications from the same institution reaches 6 applications,they will be recognized as the official delegation of that particular institution and shall be eligible for the rolling trophy.
