Children who will be age five (5) by September 30, 2016, will be eligible to enter Kindergarten in the fall.  If your child is eligible please complete the following information. This is only pre-registration, you will still be required to attend Kindergarten Registration with the proper *supporting documentation; which is scheduled Monday, March 14th through Friday, March 18th from 9:00-11:00 am, evening hours will be held Wednesday, March, 16th from 4:00-7:00 pm.  Completing this information will allow us to have your forms completed and ready for your signature at the time or registration.  You will not be required to fill out hard copies of the registration form if you complete this online form.

The registration form consists of two (2) sections.  The first section includes all the information needed to create an enrollment for your child.  The second section is the information needed to complete the Ohio Health History Form which is needed to complete enrollment.  This form will only take 15-20 minutes to complete if you gather the following information prior to starting the form: parents work and cell phone numbers; parents e-mail address; pre-school address and phone number; names and phone numbers for the child's physician; dentist; medical specialist; local hospital; and up to two (2) emergency contact's name, address, daytime phone number; a list of any medications taken and allergies.  There are two (2) pages of the Ohio Health History Form which will need to be printed and taken to your child's doctor for completion.  You are able to print these pages from the elementary school webpage. You must supply a copy of the child's current immunization record at the time of registration.

*Supporting documentation needed at registration.  In order to complete your child's registration you will need to bring the following information to Kindergarten Registration.  1) Student Birth Certificate, 2) Parent/Guardian identification such as a Driver's Licenses or State ID, 3) Custody Papers (if applicable), 4) Current Immunization Record, 5) Copy of IEP's or 504's (if applicable), 6) Proof of Residency-provide 2 of the following - Real Estate Tax Bill, Rental/Lease Agreement, Building Permit, or Real Estate Document demonstrating residence, 7) Current Gas, Electric, Water or Sewer Bill, 8) Current Bank Statement, 9) Current Voter Registration Card.

All information is merged directly onto your child's registration form, therefore it is important to use area codes for phone numbers and capitalize when necessary. It is strongly recommended that you do not start the registration process until you have the necessary information with you and you have the time to complete both sections.  If you should not be able to complete the registration at one time, you will be able to come back to update the form.
