Dear Servants,
Our beloved His Grace Bishop Serapion has requested that each servant fill out this survey. After the following questions that describe you and your service, there are three parts to the following survey.
1) the first part (questions 1-15) will assess the basic knowledge of the servant.
2) the second part (sections 3-11) focus on acquiring information of the service in each church (Sunday School, Youth meeting, spiritual days, etc.) as well as the Diocese services (Youth Conventions,  Sports leagues, etc.)
3) the third Part (section 12) provides information about the spiritual level of each servant.
All answers to these questions are anonymous, but will be used to assess the theological, pastoral, and spiritual needs of the servants of the Diocese, and help plan for the future.
