Thank you for your participation! We at the Elk Grove Village Public Library want to make our services as accessible and usable to you as possible. Your participation in this survey is very important in helping us achieve this, particularly understanding your needs and motivations for using our library services.

CONFIDENTIALITY
All your answers are confidential and will be used strictly for research. Although there may be a follow-up because of your participation in this survey, you can opt-out at any time and without penalty.

SURVEY LOGISTICS
There are about 30 survey questions and several demographic questions. The survey should take you about 10 minutes to complete.

COMPENSATION
Those who submit a completed survey will be entered into a drawing for a chance to win a prize, such as a limited edition Blackhawks hockey stick signed by #86 Teuvo Teräväinen! Providing your name, phone number, and/or email is optional, but is required if you wish to enter the drawing.
