I hereby grant National Dairy Council (NDC), Anderson Elementary, the National Football League, and each of their respective affiliates, subsidiaries, officers, representatives, employees, successors and assigns, and those acting under its authority (collectively the “Fuel Up to Play 60 Parties”) permission to publish or use still photographs or moving images, including, if applicable, any sound recordings accompanying the images (“Images”) taken of my child at any Fuel Up to Play 60 events or meetings in any and all media, whether now known or hereafter existing, and including still and moving images, without payment or any other consideration and without notifying me. I understand and agree that these Images will become the property of the Fuel Up to Play 60 Parties, which shall have complete ownership of the Images and all products created using the Images. I hereby irrevocably authorize the Fuel Up to Play 60 Parties to edit, alter, copy, exhibit, publish or distribute these Images for the purpose of publicizing the Fuel Up to Play 60 Parties’ programs, including without limitation the program entitled “Fuel Up To Play 60” or for any other lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my child’s likeness appears. Additionally, I waive any right to royalties or other compensation arising out of or related to the use of the Images. I hereby hold harmless and release and forever discharge the Fuel Up to Play 60 Parties from all claims, demands, and causes of action which I, my child, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. This release specifically includes, without limitation, a complete release and discharge of any liability by virtue of any editing, distortion, alteration, or optical illusion, whether intentional or otherwise, that may occur or be produced in the taking of or editing of said Images, unless it can be shown that they were maliciously caused, produced and published solely for the purpose of subjecting me or my child to conspicuous ridicule, scandal, reproach, scorn and indignity. I hereby certify that I am the parent or guardian of the child listed above, and do hereby give my consent without reservation to the foregoing on behalf of this person. I am over 18 years of age and am competent to contract in my own name. I have read this release before signing below and I fully understand the contents, meaning, and impact of this release.
