This form is due the Friday after the episode airs. Every student is responsible for filling this out each time we have an episode air. The highest score is a ten. Points are deducted based on whether or not you make deadline, if you turned in  your paperwork, or if you're on academic probation. If you do not submit this form, you earn a ZERO for the week.
