The link below will take you to a general survey for teachers, children and parents that has been compiled by an Education Policy Officer for WorkSafe Western Australia.

WorkSafe WA is a division of the Department of Commerce. Its primary purpose is to provide information about safe practices and to monitor safety in the workplace.

WorkSafe also provides some educational resources and activities for children and supporting information for teachers through Planet ThinkSafe, which is aimed at children between 7 and 11 years of age. WorkSafe WA is about to start a project to review and develop the Planet ThinkSafe website and resources. As part of the review, feedback from schools is sought through this online survey, with sections for teachers, children and parents.

As part of the development of the website and resources, the Planet ThinkSafe Project will work collaboratively with other organisations and services.

The information gained will only be used to provide WorkSafe WA with direction for its Planet ThinkSafe Project. Names of individuals completing the survey will not be collected.
